COMPLETE THE DIAGNOSTIC FORM
Complete the clinical diagnostic forms to be recommended by our specialists. After completion, you will be directed to book a telethealth appointment online. Specialists may request additional digital forms to be completed prior to your appointment

Receive confirmation for your appointment. As we strive to accommodate all patients amidst high demand, please ensure to confirm your appointment and submit any necessary additional documentation

  • Initial consultation: $459 (MBS Rebate: $250)
  • Review consultation: $275 (MBS Rebate: $125)

Fee Structure

  • One-off consultation: $895 
(MBS Rebate: $429.85)
  • Complex consultations and multiple diagnostic assessments will require further consultations, and pricing will differ

Previous Diagnosis from an allied health practitioner

When seeking a pediatrician’s assessment for your child, it is important to recognize that insights from behavioral reports and formal evaluations conducted by allied health professionals can provide valuable context. However, each pediatrician is an independent practitioner who will formulate their diagnosis and treatment plan based on their clinical evaluation, taking into account all relevant information. Please note that the pediatrician’s opinion may differ from diagnoses given by other professionals or those needed for NDIS funding purposes.

I want to get an ADHD diagnosis, where do I start?

To start the process of getting an ADHD diagnosis, you'll first need a referral from your GP. Click the button above to submit your referral.

I have completed the forms and booked an appointment, what happens next?

Expect to receive an SMS text message and a confirmation call from our team within the next 5 days. Additionally, you'll receive a reminder before your scheduled Telehealth appointment. If you don't receive this reminder beforehand, please contact us at 03 9052 4872 to confirm your appointment details.

What are the payment options for my appointment fees?

Payment of the total amount is required upfront at least 3 days prior to your appointment. Medicare rebate (please see our fees) will be refunded within 3 days after your appointment. We accept card payments and bank transfers whenever possible.

Do I need high-end technology for my telehealth appointment?

You don't need anything fancy. Just make sure you have a stable internet connection – it doesn't have to be super fast. Use a laptop or desktop computer if you can, with a working microphone, speaker, and camera. If not, your smartphone or tablet will work fine as long as you have internet (WiFi or mobile data). Open a fresh web browser with no other tabs to avoid technical issues. Try to join a few minutes early in a quiet, private space where you won't be interrupted for about an hour.

How do I connect for my appointment?

When it's time for your appointment, simply click on the personal link provided in your appointment reminder email or SMS. Once you're there, you'll see a 'Join meeting' screen. Just enter your name and click "Join" to start your appointment. It's that easy!

What happens if I/the patient can no longer attend?

Please contact Telecare and inform our staff as soon as possible. If this cancellation is within 48 business hours of an appointment, a $150 late cancellation fee will be charged.
